CARMEN HOUSE is a Congregate Living Health Facility specializing in recurring, intermittent, extended, and skilled nursing care. We deliver a wide array of inpatient services, including medical supervision, 24-hour skilled nursing, and supportive services such as pharmacy, dietary, social recreational, and physical and occupational therapy.

Approved by the California Department of Public Health (License # 550004722), our facility caters to mentally alert individuals, those with physical disabilities, including ventilator dependence, as well as those diagnosed with terminal or life-threatening illnesses, and those who are catastrophically and severely disabled.

With a commitment to comprehensive rehabilitation services and round-the-clock skilled nursing and supportive care, we prioritize individual patient needs. Our dedicated team, featuring a 1:2 staff-to-patient ratio and 1:1 therapy, comprises physicians, nurses, dietitians, physical, occupational, speech, and respiratory therapists, along with case managers, social workers, neuropsychological specialists, and activities directors, ensuring personalized programs and care.

We welcome private pay, worker’s compensation, Veterans benefits and most insurance plans for payment at our facility.

The ambulance driver will bring you to your room. All of our rooms are private which means you will never have a roommate. Once you are settled in your room, your nurse will begin his/her assessment of your care needs.

We are proud to collaborate with numerous local specialists who are affiliated with nearby hospitals and regularly visit our facility. Our physicians are available around the clock and maintain direct communication with our nursing team, guaranteeing the delivery of high-quality care at any time of day or night.

YES. At CARMEN HOUSE, nursing staff and certified nurse’s aides are always available to assist and evaluate our patients. Nurses administer treatments prescribed by physicians and manage medications, while our certified nurse’s aides are on hand to help with all aspects of daily living activities.

Therapy at CARMEN HOUSE is prescribed by physicians, with the frequency of sessions tailored to each patient’s personalized care plan. Our Therapy Team provides Respiratory, Physical, Occupational, and Speech Therapy services up to seven days a week. Upon admission, patients undergo screening and treatment by our therapy department, ensuring they reach their highest level of functioning and receive the necessary guidance and resources to achieve safety and independence.

CARMEN HOUSE strives to balance personalized care and independence. While we maintain a strong healthcare environment, we also run programs and invest in facilities that allow our patients to do what they love. Our accommodations are made to resemble home. But what makes our sub-acute facility feel more like home than a hospital is our warm and helpful staff. We share a belief that treating each patient like family contributes to a satisfying experience and speedy recovery. We do all these, day in and day out, to achieve one goal: to help our patients reach optimal health, strength, and confidence to resume living independently.

The goal of nursing care facility is to return your loved one back to their prior functioning. However, depending on the extent of their illness or injury, this might not be fully possible. Instead, treatment will aim to bring your loved one to the highest level of functioning possible. Keep in mind that adjustments may need to be made; they may not be able to return to a completely independent living situation.

Whatever the future may hold, CARMEN HOUSE will work with you and your loved one to help you understand the next steps. If long-term treatment is required, we will be here for you.

Whether it’s a short stay to help with rehabilitation from a bone fracture, heart attack or stroke, or a long term stay to assist with daily ventilator or tracheostomy care, CARMEN HOUSE has you covered.

Every person is different. The duration of your stay is determined by the progress you are making, evaluation by your insurer, and discharge goals. Plans for discharge are made in conjunction with both the patient and family members. Some patients have been with us for more than five years.

CARMEN HOUSE does not impose restrictions on visiting hours. We welcome guests at all times, provided that their visits do not interfere with the care and well-being of our patients.

Yes, dogs or pets are welcome as long as they are leashed. However, they are not permitted in the main dining area. Visitors can bring their pets and utilize other designated visiting areas at any time.

We recommend reaching out to our Administrators for assistance in organizing a party for your loved one. They’ll be happy to discuss the details with you. Additionally, feel free to bring along any party items you’d like to include.
